Supported Employment Program Manager:
 Direct the development, implementation, and evaluation of supported and supportive work, transition to work, and competitive community employment programs.   Supervise employment trainers, job coaches, and community trainers.  Promote and maintain good relations with parents, supporting agencies, and employees.  Coordinate with Consumer Services Manager on consumer intake admission process.  Ensure that case records are maintained in accordance with 2390 regulations.  Ensure that support services are developed and provided to workers and their families.  Advise and assist parents, guardians, and workers with Social Security Administration guidelines and inform them as how employment may affect their social security benefits/eligibility.   Ensure that required placement records are maintained. Ensure that monthly follow-up contact is maintained with all workers once intensive on-the-job training is completed.  Assist in the development of new positions for placement.  Represent agency employment initiatives in overall Job Development with community based corporations and companies.   Ensure that a complete and thorough job analysis is completed for each position to ensure a proper job match with worker's skills, strengths, and weaknesses.  Ensure that data collection activities for any program evaluation are carried out.  Act as liaison for Community Placement Services for agency to funding sources and employment training community.   Participate in agency activities and committees as assigned.  Provide investigative support for unusual incidents.  Assist with the annual budget submission for Placement Division.  Monitor the placement budget on a monthly basis.  Other duties as required.  MINIMUM QUALIFICATIONS AND PREVIOUS EXPERIENCE:  Bachelor's Degree in field of Social Services or Business.  Previous experience as job coach/employment trainer. Successful job development for supported employment programs, Experience working with developmentally disabled.  Minimum five years supervisory experience.
 
Facility Maintenance Manager: 
 Responsible for completing major and minor repairs of facility to include plumbing, heating, air conditioning, electrical and other systems and equipment. Monitors all building systems and equipment for proper and safe operation and ensure preventive maintenance as required.  Supervises custodial staff and monitors custodial operations and special cleaning projects.  Maintains adequate supply of cleaning and maintenance supplies and equipment.  Performs periodic inspections of exterior and interior of building for repairs and safety.  Maintains fire safety and detection equipment.  Conducts monthly inspection of fire alarms.  Monitors work performed by outside contractors and consultants.  Responsible for maintenance of all agency motor vehicles.  Operates agency motor vehicles when required.   Provides reports as required by the Director of Operations and Programs. Responsible for snow and ice removal from sidewalks, walkways, and parking lot.  Participates in agency committees as assigned. Maintains/monitors life/safety/housekeeping in accordance with all governmental regulations. Responsible for building security. Ability to accommodate flexible hours. Performs other duties as required.  MINIMUM QUALIFICATIONS: Requires HVAC experience plus routine maintenance, plumbing, electrical, and carpentry and painting ability.  Some supervisory experience of janitorial and/or housekeeping staff required.  Valid motor vehicle operator's license and good driving record required.Must be High School graduate or GED certified.

 

SERVICES COORDINATOR:

Case Managment - Monitors consumer programs through the Individualized Service Plans (ISP).   Serves as liaison for internal referral and to other community agencies.  Coordinates the interdisciplinary team and works in assuring quality and appropriateness of their services.  Provide supervision and training to direct care staff ensuring understanding of and proper documentation of consumer program implementation. Organize, maintain, and monitor consumer files in assigned caseload.   Coordinate all consumer services and program supports for assigned caseload.  Schedule and announce ISP review meetings, team conferences, and other meetings involving consumer services.  Utilize Agency consumer data base for program planning and ensure outcomes are measured annually.  Develop and coordinate the planning process for the ISP with the consumer’s primary support staff at least five days prior to the meeting.   Monitor consumer program daily to ensure correct implementation of ISPs.  Develop and enter ISP monthly and quarterly reports in consumer files.  Obtain consumer and staff signatures on all documents subsequent to review. Assure accuracy of information in client files through monthly reviews and evaluation of all essential data (data sheet, goal plans, monthly and quarterly reports, and ISPs).  Disseminate ISPs and reviews to team members, OMH/MR case manager, commonwealth advocates, consultants, residential programs, and families. Maintain case records, which include case identification data, reports of service, referrals, and ISPs.  Review incidents daily and report all unusual incidents to appropriate agencies within 24 hours of occurrence.  Assist in annual licensing survey and maintain accuracy of consumer files for all monitoring and licensing surveys. Serve as liaison between the agency, families, and other community agencies. Conduct staff and consumer training as required. Attend meetings, training, and orientation. Develop and write goal plans in accordance with ISPs. Act as liaison between agency and transportation provided by families. Intervene in crises situations. Other duties as required. MINIMUM EXPERIENCE AND TRAINING REQUIREMENTS: Bachelor’s Degree in Rehabilitation, Psychology, Human Services, Social Services, or related field.  Five years experience in Human Services.  Valid vehicle operator’s license.


Junior Counselors :  Camp PEP - temporary position - July 7th through August 1st.  Mon-Fri 8am -3pm.  general duties include supporting children and young adults with disabilities in summer camp activities, attend trips, movies, sporting events, swimming, arts and crafts and music programs.  provide guidance and supervision. A great entry level position for people interested in helping others with different abilities and who know how to have fun and make learning a fun and rewarding experience.   good job for high school and college student.
