  Ulster County Clinic Peer Advocate

Position Summary:

Serve as Peer Advocate and Clinic liaison for consumers of Ulster County Mental Health clinic services in Ulster County. This position is employed by PEOPLe, Inc. and co-supervised by PEOPLe, Inc. and UCMH. Services are provided at Ulster County clinic locations in Kingston, Highland and Ellenville with office space in Kingston. The Peer Advocate will provide individual support and advocacy, offer education and outreach about recovery and wellness to both clinicians and consumers, and facilitate a variety of groups related to recovery and wellness.

Responsibilities:

·         * Represent the mission and goals of PEOPLe, Inc.

  *  Provide peer support and advocacy for clinic participants, assisting in systems navigation and the achievement of person-centered recovery and wellness.

·         * Provide individual and/or systems advocacy services to clinic participants, both internal (within UCMH) or external, acting as a mediator in clinical relationships if requested.

·         * Making referrals to community services as needed or acting as liaison to local services/programs and community supports.

·         * Encourage and support participants in learning and practicing tools of recovery and wellness, including WRAP, Advanced Directives and other tools.

·         * Maintain timely and accurate documentation of all participant contacts and professional activity. Compile and submit program statistics and reports as directed.

·         * Facilitate or co-facilitate a variety of on-site recovery and wellness-oriented groups.

·         * Provide outreach and education about recovery and wellness to mental health service providers, consumers of mental health services and the community at large.

·         * Actively participate in regular staff meetings and community committees/projects as assigned.

·         * Other duties as assigned.

Competencies:

·        *  Experienced advocate with knowledge of community resources.

·         * Experience and/or training in group facilitation.

·         * Demonstrated planning and organizational skills.

·         * Detail-oriented and able to keep accurate, timely records.

·         * Excellent communications skills (oral, written and computer).

Requirements:

·         * BA or equivalent in related field.

·         * Two years experience in human services or related area.

·         * Current or past recipient of mental health services preferred.

·         * Valid, clean NYS drivers license.

Hospital Diversion House Peer Companions
-- Multiple Opportunities
 
       A Peer Companion is available to guests staying at our Hospital Diversion House at all times to provide peer support and companionship for guests. We are seeking Peer Companions for our existing hospital diversion house in Milton, NY (Orange County) as well as for a forthcoming hospital diversion house in Putnam County. Please indicate your preferences in your cover letter.
 
       The Peer Companion is available to instill hope, to support wellness and to demonstrate that recovery is real and does happen. Peer Companions should discuss the WRAP Plan as well as an Advanced Directive. Other important subjects are goals, community and social events.

 
Essential Duties and Responsibilities include the following. Other duties may be assigned.

* Provide supportive companionship for guests during interview, intake, arrival and departure.

* Share personal experience and encourage mental health wellness and recovery.

* Assist in connection to mental health services, community resources, entitlements, and other services as needed.

* Help maintain a clean, safe and secure environment. Some cleaning duties may be assigned.

* Transport guests to store or appointments when needed, using company car.

* Provide telephone support to peers in the community.

* Keep accurate and timely records.

* Work cooperatively with Peer Services Coordinator, House Manager, and Team.
