Care Management Program

The Care Management Program addresses the multi-faceted needs of older and frailer Upper West Side senior neighbors. OSSS creates individualized care management plans and conduct home visits for clients who need intensive services. The program’s objective is to put all necessary services in place in order to promote the mental and physical well-being of these especially needy individuals so that they can live safely and independently in their own homes for as long as possible.
