Most Innovative Caregiving Practitioner
Recognizing the work of professional caregivers
 
Deadline for Submissions: October 15, 2009
 

Background: In recognition of the vital role of individual professional caregivers in meeting the health and human services needs of the citizens of Harris and surrounding counties, Senior Guidance Directory’s Board of Directors created the Pacesetter Award.   This honor recognizes the extraordinary achievements of those who search out, arrange, or otherwise provide for the critical needs of our community's most vulnerable citizens while ensuring dignity, independence and self-sufficiency. Presentation of the Pacesetter Award will be made during a luncheon to be held on Thursday, October 29, 2009.  Further information about the luncheon may be found here.

Nomination Form
ELIGIBILITY
Nominees for the award are those individuals who go above and beyond in providing professional caregiving services.  An employer, private individual or family member of a senior may submit nominations.

Candidates will be considered based on the following:

  • Demonstrating innovative caregiving techniques.
  • Having a measurable history of giving time and talent to Houston’s seniors.
  • Being a role model for others in the area of caregiving through ethics and professionalism.
  • Valuing the input of others while working in collaboration to accomplish the best possible outcomes for seniors.

General Guidelines:

  • Individuals must provide services within Harris or surrounding counties, either currently or within the past six months.
  • Nominees may be individuals or organizations, public or private.

Submission Requirements:

  • Nominations are not to exceed two pages, typed, which describe specific reasons for the candidate’s nomination. Additional letter(s) of support may be included. If an individual is nominated, the candidate’s resume or curriculum vitae must be submitted and should include information on the individual’s career, background, volunteer efforts, prior awards and other notable achievements.
  • Members of the Senior Guidance Directory Board of Directors and Advisory Council will review nominations and their decision will be final.  Nominations are due no later than 5:00 pm, September 15, 2009.
  • Presentation of the Awards will take place at the Solving the Caregiving Puzzle…One Piece at a Time Luncheon on Thursday, October 29, 2009.
