Lee County Physical Activity and Nutrition Coalition (LeePAN) 
LeePAN is a community coalition of agencies and individuals whose mission is "to positively impact health in Lee County by encouraging individuals, businesses and organizations to promote active living and healthy eating through education, policy and environmental change."  The Health Planning Council of Southwest Florida is actively involved in LeePAN (see Calendar for LeePAN committee meetings and events).   LeePAN was involved in a pedometer program encouraging through a grant to the Lee County Health Department.  HPCSWF and the LeePAN have developed a Guide to Healthy Living in Lee County with tips and resources on healthy eating and activity in Lee County.   
 
County Profiles 

The Health Planning Council of Southwest Florida and the other 10 health councils around the state have developed County Profiles for each county in their district.   The profiles are a snapshot of the health status and related indicators for public officials and community partners.  The following for the county, district and Florida is included:

  • Demographic data by age, race, ethnicity and gender
  • Socioeconomic date including income, poverty, unemployment and education
  • Maternal and infant health data including birth rates, low birth weight, teen births, prenatal care, immunizations and infant death
  • Health access information including percentage of persons uninsured
  • Federal designations for professional shortage areas
  • Leading causes of death including rates by type
  • Utilization data including hospital discharge rates and bed days and nursing home utilization rates

Disaster Preparedness for Persons With Disabilities 

In response to the Florida Department of Health (DOH), Public Health Preparedness Strategic Plan, the Taskforce for Persons with Disabilities and Preparedness (TPWDP) requested a regional census and survey of Persons with Disabilities (PWD).  The census and survey will serve as the basis for future planning to recognize and accommodate PWD, particularly during a catastrophic event in which this population may be more vulnerable.  Persons with disabilities account for as much as 20% of the state’s population.

The Local Health Councils (LHC), a network of eleven non-profit agencies enacted by the Florida Legislature operate as an arm of the state Department of Health to conduct regional health planning and implementation activities to improve access to health care, reduce disparities, and assist local governments in developing health policies. Four Local Health Councils have agreed to participate in this project: Broward Regional Health Council (Broward County), Health Council of South Florida (Miami-Dade and Monroe Counties), Health Planning Council of Southwest Florida (for only Charlotte, Collier, Hendry, Lee, and Sarasota Counties), and Treasure Coast Health Council (Indian River, Martin, Okeechobee, Palm Beach, and St. Lucie Counties). 

Over the summer of 2006, the Persons With Disabilities (PWD) Committee of Southwest Florida collected data on more than 600 persons with disabilities.   In December 2006, the Broward Regional Health Planning Council produced the Disaster and Emergency Preparedness: Report of the 2006 Regional Census and Survey of Persons with Disabilities.  

The Charlotte County Health Services Access project is a strategic planning project focused on finding solutions to providing affordable, accessible, and appropriate levels of the continuum of care to the uninsured in Charlotte County.   The ChCHSA Consortia meets monthly on the 4th Monday of the month at the Charlotte County Government building (18500 Murdock Circle, Port Charlotte). 
 

DeSoto County MAPP Project

The Health Planning Council of Southwest Florida is participating in the MAPP process in DeSoto County.  MAPP stands for Mobilizing for Action through Planning and Partnerships.  Meetings are the second Friday of the month at the DeSoto County Health Department.  

Glades County MAPP Project

The Health Planning Council of Southwest Florida is facilitating the MAPP process in Glades County.  MAPP stands for Mobilizing for Action through Planning and Partnerships.  Meetings are the third Friday of the month (9AM to 11AM) at the Moorehaven Public Library.  

District 8 Health Plan 2003-2006

The Health Planning Council of Southwest Florida, under the contract with the Florida Department of Health, developed the 2003-2006 District Health Plan.
