By completing this form with your name, phone number and mailing address, along with identifying in the comments which supermarket you want us to sign you up for--Raleys, Save Mart, and/or SPD--we will get you a card and mail it to you for you to use every time you shop. Using this program allows 3% of every receipt to be donated by the Supermarket to our agency! There is absolutely no cost to you. Details can be found on the Donate page of our website.
